About the fund holder

The Kenneth Molloy Fund has been established to allocate funds to support individuals and groups who are linked to the Haven Surgery, Burnhope.

What we will fund

Applications for up to £250.00 (higher amounts may be considered in exceptional circumstances) for items/activities which will make a significant impact on people’s health and wellbeing, for example:

• Travel costs relating to medical or hospital visits
• Respite care
• Domestic equipment e.g. fridge, washing machine, cooker
• Specialist equipment, e.g. communication, mobility, sensory
• Projects to improve health by healthy eating, healthy living, fitness, exercise and education. If you are applying on behalf of a group please follow this link to apply on our standard expression of interest form.

What we will not fund

• Any activity which is the responsibility of the local or health authority.
• Retrospective applications i.e. anything that has already been paid for.
• Salaries (although volunteer expenses may be considered).

Are you eligible?

If you are an individual you must be a registered patient at the Haven Surgery in Burnhope.
For groups the majority of your beneficiaries must be registered patients at the surgery.

IMPORTANT: The application should be completed by the individual applying for the grant. If this is not possible, please ask someone else to help you e.g. a parent/your referee.

You will also need to include two references (please use the reference forms provided) one from a health care professional attached to the Haven Surgery and one from someone who knows you and can support your request, but is not a member of your family or a close friend. Please include a telephone number and ask permission for us to contact him/her if necessary.
